This site holds your full plan details and documents. When you’re ready to enroll, use either option below.

1

Option one: enroll over the phone

Option two: enroll in Paycom

  1. Start. On the Benefits tile, click Benefit Enrollment. The Progress Box on the right tracks each plan — a green check means enrolled, a red X means declined.
  2. Confirm your details. Select Start Enrollment, review your personal information, and add any dependents (Social Security Number and date of birth required). Click Save and Next.
  3. Choose your plans. Accept or decline each plan, pick your coverage level and covered dependents, and add beneficiaries where required (such as life insurance). You can exit at any time — your progress is saved.
  4. Finalize. Review your selections, click Finalize, then Sign and Submit. Any plan you don’t enroll in is declined. Print your confirmation from the Benefit Confirmation page.

After you enroll: view or print your coverage anytime under Current Benefits on the Main Menu. For a life change such as marriage or a new dependent, go to Qualifying Events on the Benefits tile, add the event and date, and upload supporting documents (for example, a marriage or birth certificate).

Notices

Medicare Part D

This notice confirms that your prescription drug coverage is creditable, which means it does not prevent you from enrolling in Medicare Part D if you wish. The open enrollment period for Part D is November 15 through December 31. Delaying Part D enrollment will not result in higher premiums as long as you are covered by a creditable prescription drug plan.

If you are Medicare eligible, you will receive information on the Medicare Part D plans in your area. Premiums vary by location but are expected to average about $32–$37 per month. If you enroll, coordination of benefits will be required.

Section 125

You may be able to change your benefit elections during the plan year — additions, deletions, and cancellations — if you experience a qualifying event as defined by IRS Code Section 125, and only if the event affects your own, your spouse’s, or your dependent’s coverage eligibility.

You must report a qualifying event to Human Resources within 30 days of the event. After 30 days, additions and deletions will be denied, and you may be responsible both legally and financially for claims or expenses incurred by dependents who no longer meet eligibility requirements. Approved additions are generally effective on the date of the event; cancellations are processed at the end of the month.

Payroll deductions for health, dental, vision, and certain supplemental accident premiums are taken before your income is taxed under the Cafeteria Benefit Plan governed by IRS Code Section 125, which means you pay less tax on a per-pay and annual basis.

COBRA

The Consolidated Omnibus Budget Reconciliation Act (COBRA) of 1985 requires most employers sponsoring group health plans to offer employees and their families a temporary extension of health coverage (continuation coverage) at group rates in certain instances where coverage under the plan would otherwise end. An employee, an employee’s spouse, or a dependent child covered by the group health plan has the right to choose continuation coverage if coverage is lost for any qualified reason. Please speak with Human Resources for more information.

Medicaid and CHIP

If you or your children are eligible for Medicaid or CHIP and you are eligible for health coverage from your employer, your state may have a premium assistance program that can help you pay for coverage. These programs help people who are eligible for Medicaid or CHIP but also have access to health insurance through their employer.

If you are not currently enrolled in Medicaid or CHIP and think you or a dependent might be eligible, contact your state Medicaid or CHIP office, call 1-877-KIDS NOW, or visit www.insurekidsnow.gov to find out how to apply. If you qualify, you can ask the state whether it has a program to help pay the premiums for an employer-sponsored plan.

The materials on this site provide a brief summary of the benefits offered and are not contractually binding. For complete information, please refer to the certificates of insurance. In the event of a discrepancy or omission, the carrier policy will govern.
