Elections you make during open enrollment will become effective July 1, 2026. If you are a new hire, your elections will be effective on the first of the month following 60 days from your date of hire.

All three plans include free virtual care through Sam by UCM Digital Health for PCP, specialist, behavioral health, ER, urgent care, and nutrition. Submitting documentation — manage your account and submit Benefits Card receipts, EOBs, or invoices through the My Mobile Account Assistant app or online at ebcflex.com. If you continue your enrollment year-over-year, you will not receive a new Benefits Card — the existing card carries forward.

OptiMed is your secondary carrier — you’ll always have a primary medical plan (WellNet or Cigna) and OptiMed reimburses eligible inpatient and outpatient costs after your primary processes the claim.

What GAP covers — inpatient hospital stays, procedures, physician charges, mental health and substance abuse treatment, routine nursery care, outpatient surgery, radiation and chemotherapy, diagnostic imaging (X-ray, MRI, CT, PET, EKG, stress tests, cardiac catheterization), emergency room visits for accident or illness, urgent care for accident or illness, and kidney dialysis.

Eligibility

Full-time team members working a minimum of 32 hours per week and their eligible dependents may participate in Page Autos benefits. Eligible dependents include your legal spouse or domestic partner, children up to age 26, and unmarried children of any age who are not able to support themselves due to a mental disability, physical disability, mental illness, or developmental disability.

Under Florida Statute §627.6562, dependent children residing in Florida may remain on a parent’s group health plan through the end of the calendar year in which they turn 30, provided the child meets all of the following conditions: is unmarried and does not have a dependent of their own; is a Florida resident or a full-time or part-time student; and is not covered under any other group or individual health plan or entitled to Medicare benefits.

Qualifying Life Events

Changes to benefit elections outside of open enrollment require a qualifying life event reported to HR within 30 days:

  • Marriage or divorce
  • Birth or adoption of a child
  • Change in a child's dependent status
  • Death of a spouse, child, or qualified dependent
  • Change in employment status affecting coverage under another employer-sponsored plan

Frequently Asked Questions

When do my benefits start?

The current plan year is July 1, 2026 – June 30, 2027. If you're a current eligible employee, your coverage continues throughout the plan year. If you're a new hire, your benefits start after your applicable waiting period of 60 days, and coverage is then effective First of the month following. See the New Hires section for details on what you'll need to enroll.

What if I miss the enrollment deadline?

If you miss your initial enrollment window, you'll need to wait until the next Open Enrollment — or experience a qualifying life event (marriage, birth, etc.) within the past 30 days. Outside of those windows, mid-year benefit changes generally aren't allowed.

Where do I find my insurance ID cards?

Most carriers offer ID cards through their member portal or mobile app within a few weeks of your enrollment. You can also reach out to the carrier directly using the contact information in the Carrier Directory. If you can't locate yours or have questions, contact your Benefits Team for help.

What happens to my benefits if I leave?

When your employment ends, your benefits will continue through the end of your termination month or until the date specified by your plan. You may be eligible to continue coverage through COBRA — your HR team will provide details and election forms. Reach out to your HR team for plan-specific information.

For claims questions, contact your carrier directly using the phone number in the Carrier Directory. For help understanding your benefits, deductible status, or what's covered, your Benefits Team can also assist.

New Hires

Full-time employees working 30 or more hours per week are eligible for benefits upon satisfying the applicable waiting period of 60 days. Coverage is effective First of the month following following the end of your waiting period.

You have 30 days from your benefits eligibility date to enroll. Missed deadlines require waiting until next open enrollment or a qualifying life event.
